    Amergis Healthcare Staffing is partnered with a much respected facility in Amherst MA who is currently seeking Peer Counselors to assist them at their Independent Living Facility.


    Details of the position include:

    Location:
    Amherst, MA

    Duration: 13 weeks to start, but strong chance of extension


    Facility Type:
    Independent Living Facility


    Openings:
    8a-6p Mon-Fri

    Start Date:
    Two weeks from offer


    POSITION DESCRIPTION:
    Conduct LTSS assessments; by identifying the needs and recommending services and resources

    Connect consumer to social services and community resources

    Participate on the consumer's care team

    Perform outreach

    Advocate for the member

    Provide LTSS care coordination

    Initial and quarterly home visits

    Remain current with community resources


    QUALIFICATIONS:
    Must have knowledge of community resources and services available for people with disabilities.

    Excellent computer skills, knowledge of MS Office and database navigation

    Excellent communication and writing skills to ensure respectful delivery of services

    Excellent time management and organizational skills

    Must have valid MA driver's license and reliable transportation as well as proof of insurance if applicable.
