    Job Overview

    High Care Palliative is seeking a dedicated and compassionate Nurse Practitioner to join our team in delivering high-quality palliative care services to patients in their homes.

    As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to provide comprehensive care that addresses the physical, emotional, and spiritual needs of individuals with chronic illnesses.

    About the Role

    We are offering a competitive salary of $120,000 per annum, commensurate with experience, plus benefits.

    This is a full-time position requiring regular travel within the community to visit patients in their homes. A valid driver's license and reliable vehicle are essential.

    Responsibilities

    • Conduct thorough assessments of patients' medical conditions and develop personalized care plans in collaboration with other healthcare professionals.
    • Administer medications and treatments as prescribed by physicians, while monitoring patients' responses and adjusting care plans accordingly.
    • Provide education and support to patients and their families on managing symptoms, using medication safely, and navigating the healthcare system.
    • Maintain accurate and detailed records of patient interactions, progress notes, and treatment outcomes.

    Requirements

    • Current Registered Nurse license and unrestricted Nurse Practitioner license issued by the state.
    • Current DEA Certificate and Basic Life Support (BLS) certification.
    • Experience working in hospice, palliative, or home health settings, or a similar field.
