GCBHS has immediate openings for a ResiliencyCase Manager to provide case management (community psychiatric support treatment) with a focus on assisting our co-occurring mentally ill and developmentally disabled (MI/DD )clients to develop Resilience factors in the community in order to help them live as independently as possible

    We are looking for BSW or Psychology or Sociology or Criminal Justice or related Human Services degree graduates.     Responsibilities for ResiliencyCase Manager:

    • Provides services accessing community resources, advocacy, support and collaborative follow-up with identified referral sources through face-to-face and telephone service as needed to assist individuals and families with meeting established goals.
    • Performs a variety of case oriented duties, including case documentation, review of clinical assessment information to assist with client evaluation.
    • Attends/coordinates Trauma Informed Biographical Timeline meetings, Trauma informed supervisory meetings, and other program or unit meetings as required, participating in discussions regarding clinical, program, and administrative issues.
    • Works collaboratively with team including, therapist, DD worker, and community providers to promote resiliency within the client.

    Requirements for ResiliencyCase Manage r:

    • BA in Psychology, Social Work, or a related Human Services field or a minimum high school diploma plus 3 years experience in mental health.
    • Position requires frequent driving of own personal vehicle. A valid driver's license, vehicle, and insurable driving record are required.
    • Position requires working with individuals in both Hamilton and Clermont Counties.
    • Candidates with experience working with Individuals with developmental disabilities preferred
