CONTRACTOR QUALIFICATIONS AND EXPERIENCE:

The Project Manager, 1 Team Leader assigned to this agreement must possess, at minimum, the following

• qualifications, and experience:
• At least ten years of clinical Critical Care and Emergency Medical Services (EMS) experience.
• At least ten years of experience as an Air Medical Crew or Paramedic Instructor.
• At least an associate degree in nursing or EMS.

CONTRACTOR RESPONSIBILITIES:

Contractor provide a Project Manager, 1 Team Leader to the Department throughout the purchase order term that will perform the following tasks in the time and manner specified:

• Ensure the Project Manager, 1 Team Leader reviews all deliverables and invoices for compliance with the criteria and payment schedule provided for in purchase orders and contracts associated with successfully managing the implementation of a healthcare communications and patient logistics platform as directed by the Department.
• Ensure the Project Manager, 1 Team Leader visits the physical location where the services are delivered and speaks directly to the staff responsible for delivering the services associated with successfully managing the implementation of a healthcare communications and patient logistics platform as directed by the Department.
• Ensure the Project Manager, 1 Team Leader meets at least twice a month with the Department Contract Manager as directed by the Department. Ensure the Project Manager, 1 Team Lead maintain records of such meetings as directed by the Department.
• Ensure the Project Manager, 1 Team Leader provides direct support to all Florida licensed EMS agencies for the implementation of a healthcare communications and patient logistics platform as directed by the Department.
• Ensure the Project Manager, 1 Team Leader provides direct support to all Florida licensed hospitals for the implementation of a healthcare communications and patient logistics platform as directed by the Department.
• Ensure the Project Manager, 1 Team Leader provides direct support to all Florida emergency management agencies for the implementation of a healthcare communications and patient logistics platform as directed by the Department.
• Ensure the Project Manager and 1 Team Leader conducts additional activities as directed by the Department.

DATA SECURITY AND CONFIDENTIALITY TASK:

• The Contractor, its employees, subcontractors, and agents must comply at all times with all
• Department data security procedures and policies in the performance of this scope of work as specified in the Data Security and Confidentiality document attached to the purchase order.

DELIVERABLES:

Contractor will complete and submit the following deliverables to the Department in the time and
manner specified:

• Monthly: Provision of a Project Manager, 1 Team Leader to manage the implementation of a healthcare communications and patient logistics platform in the time and manner specified in Tasks
