About the Role

• We conduct adult/geriatric assessments, including medical history diagnosis and treatment, health education physician referrals case management referrals follow-up and clear documentation according to Matrix guidelines and protocols.
• Our providers administer point of care lab-based testing as needed work with physicians case managers social workers family members key caregivers ancillary medical personnel collaborate with Primary Care Physicians on patient education follow up as necessary provide services at the member's place of residence which can be in their homes or Skilled Nursing Facilities.
