    WAGE RANGE: $22.50-$26.30/hour

    JOB SUMMARY: Catholic Charities is seeking a self-motivated advocate who is compassionate, trauma-informed, and culturally aware to serve as a Case Manager II at its Minneapolis Opportunity Center location.

    They will work directly with assigned clients to develop the client's service plan, including the intake assessment of clients' needs, available community partners and resources, and applicable Catholic Charities services. They will adhere to case documentation requirements and work directly with clients to support client success, track client progress and adjust service plans as necessary.

    They will serve as an advocate for the client and provide education related to services and challenges to the community.

    This position requires the candidate to have a valid Minnesota driver's license, a reliable vehicle, and be able to provide proof of insurance.

    ESSENTIAL FUNCTIONS:

    1. Completes basic assessments, prepares, evaluates and maintains a service plan to meet the needs of the client including both Catholic Charities programs and other available community resources.
    2. Supports and advocates on behalf of client in the procurement of services and benefits entitlement, assisting with required paperwork as necessary.
    3. Makes referrals to and coordinates appropriate community resources for client.
    4. Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies.
    5. Participates in the education of the community, volunteers, service providers, and other interested parties with respect to client services and programs.
    6. Coordinates with the supervisor on all aspects of client care and seeks appropriate guidance and feedback from other team members.
    7. May transport clients.
    8. Consults and cooperates with community systems to facilitate linkage, referral, crisis management and follow up, focusing on attaining goals.
    9. Provides assessment, education and case management services to a complex, often long-term group of assigned clients, which may include in-home visits.
    10. Advocates for clients in procurement of services.
    11. Other duties as assigned

    MINIMUM QUALIFICATIONS/EXPERTISE:

    • Bachelor's Degree in human services or related field preferred with minimum of two (2) years of experience working with a specific client group.
      1. In lieu of degree, a minimum of three (3) years of experience working with a specific client group is required.
    • Must have a valid driver's license and proof of current insurance coverage if transporting clients.
    • Ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness required.
    • Additional training, education, or licensure may be required per program need.
