    DESCRIPTION:

    The Community Resource Center is a unique department at SAC Health that aims to address health inequities within the community such as lack of access to care, food insecurity, homelessness and housing challenges, among many other issues, in the San Bernardino region. All staff of the department is required to have basic knowledge of the Social Determinants of health, and its impact in underserved communities. Duties will include, but not limited to, organizing and overseeing information and resources provided by staff to the community. Incumbents of this position will receive direction from the department head or supervisor. This position will spearhead the CRC programs and events and will be required to provide monthly reports and ensure the CRC quarterly newsletter is completed. Tasks include maintaining accurate documentation, sustaining community partnerships, and teaching community classes. Performs other duties as assigned. This role is responsible for participating and assisting the CRC in continuous improvement activities at SAC Health. Collecting information, analyzing and communicating information to accomplish goals as outlined by the department head.

    Schedule: 10 hour days, 4 days per week

    ESSENTIAL FUNCTIONS AND DELIVERABLES:
    • Assists with program delivery services and ensures the completion of assigned projects tasks, within scheduled periods, following documented departmental procedures.
    • Plans, assigns, reviews work/case management of staff who are directly engaged in the delivery of social services and resources; consults with and guides staff in conducting intake interviews, resources, support and guidance to clients with complex or specialized needs. Required to see patients when necessary.
    • Serves as a coach and mentor to new staff, providing mock interviews and acting as an observer during intake interviews while staff is on probation.
    • Works directly with the CRC Supervisor to find projects that need to be implemented and completed.
    • Incorporates the Social Determinants of Health (SDoH) when seeking to address the needs of clients and ensures the same for all staff members.
    • Leads departmental events and programs focused on serving the community and bridging the gap of care, under the direction of the CRC Manager. Attends all SACHS events hosted by the CRC.
    • Collaborates across interdisciplinary work teams, in conjunction with peer team leaders, clinical champions, and subject matter experts.
    • Works directly with teams to resolve issues to ensure success and timelines are met on projects.
    • Oversees outreach projects, health fairs, and ensures participation of the department in community-based meetings on a regular basis.
    • Other related duties and responsibilities as assigned.
    QUALIFICATIONS:
    • Education: Bachelor's Degree in Public Health, Social Work, Human Services or another related field is required.
    • Licensure/Certification: Requires a valid current California driver's license, current motor vehicle record and proof of auto insurance. Must maintain a good driving record and current auto insurance. As a requirement of this position, you must receive EPIC certification for the module you have been hired into.
    • Experience: Minimum three (3) years of clinical and or case management experience. Experience with community education and groups preferred. Minimum of 6 months of experience working with individuals who are homeless or experience mental health issues as a Community Resources Specialist II. Bilingual skills in English/Spanish. Experience in an outpatient clinic preferred. Essential Technical/Motor Skills: The ability to use project planning software, spreadsheets, and word processing software; input data, typing, copying, and effective use of the telephone. Experienced with Microsoft Outlook, Excel, PowerPoint, and Publisher.
    • Interpersonal Skills: Strong planning, problem solving, documentation, and organizational skills. Ability to analyze and interpret data, analyze and interpret processes and needs; define problems and solutions. Able to manage time effectively and plan and implement objectives effectively. Ability to maintain a professional attitude in difficult situations and communicate effectively. Understands needs of clinic and works well in a team environment. Must have the ability to be assertive. Demonstrate leadership skills. Ability to balance administrative responsibilities, client needs, and task management skills are required. Organized and detail oriented. Good presentation skills. Excellent communication skills
