    Description


    Director of Nursing will supervise the overall clinical operation of hiring, training/developing, and directing staff in the provision of the agency service, chart audits and filling clinical functions in compliance with agency policy and procedure, state regulations, and physician's orders.

    Directs day-to-day office/clinical operations, and all field staff. Highly organized, detail-oriented, and possess effective communication skills both orally and in writing.

    Fair understanding of the issues related to the delivery of home health care services and be able to problem solve.

    Previous supervisory experience in a home health setting is strongly preferred.

    Basic job responsibilities will include but are not limited to the following:


    • Quality assurance including reviewing charts and notes
    • Case management
    • Coordinating referrals
    • Staff education and in-services, formulates and delivers educational program
    • Staff preparation for the OASIS
    • Oasis Knowledge to include hands-on experience with review
    • Reviewing OASIS for correct billing information
    • Reconciles census
    • Monitoring visits with nursing staff two to three visits a week
    • Manages or resolves escalated and elevated concerns and issues
    • Manages the quality and timeliness of daily functions through scheduling, monitoring attendance, and supervising assigned staff
    • Knowledge of leading practice in clinical care and payor requirements
    • Ensures accuracy of financial reporting and supports clinical billing leadership
    • Coordinates audit requests from internal and external sources
    • Self-motivated, proven communication skills, assertive
    • Consults as needed with the physician and physician office regarding patient care
    • Assuring the development, implementation, and updates of the individualized plan of care
    • Ensure thorough coordination and compilation of Quality Assurance the accurate OASIS collection
    • Background in business planning, and targeted outcomes
    • Working knowledge of managed care, inpatient, outpatient, and the home health continuum, as well as utilization management and case management
    • Working knowledge of the concepts associated with Performance Improvement And performs all other duties as assigned

    Job Requirements:

    • Active RN license in the State of MA.
    • Graduated from an approved school of professional nursing.
    • Must have a minimum of three years of nursing experience, with at least 2 years of recent supervisory experience.
    • Previous Medicare or Medicaid or Private Duty Nursing Home Health experience.
    • Knowledge of state and federal regulations governing Home Health Care.
    • Must have excellent Organizational leadership, team building and diplomacy skills.
    • Exceptional communication skills
    • Upbeat, positive attitude and the ability to work in fast-paced, sometimes high-stress environment.

    Basic computer software skills:
    Word, Excel, Power Point Internet/Intranet use.

    • Demonstrated effective working relationship with physicians.
    • And absolute love of patient care and the desire to be the best
    • Previous experience caring for medically complex patients
    • Strong interpersonal skills that facilitate the building of long-lasting and meaningful relationships with our patients/families, clinicians, providers, and vendors

    Environmental and Working Conditions:

    • Performs duties in an office environment.
    • If required to make patient visits, may work in patients' homes in various conditions.
    • Possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather

    Personal Development:
    Coordinates, monitors and meets agreed-upon goals and objectives through establishing individual performance expectations.

    Ensures staff performance meets or exceeds expectations by providing regular feedback and conducting annual performance appraisals on a timely basis.

    Reviews and monitors team needs for hiring, orientation, training, and retention of a sufficient number of qualified staff.
    Identifies skill development needs to manage employee development.

    Provides feedback to Training department and applies techniques to coach and motivate the performance of the team and individual staff.

    Identifies training needs and provides staff training, including cross-training opportunities.


    Customer Service:


    Monitors and manages the process of providing exceptional service by understanding customer needs and responding to specific and general inquiries received from internal and external customers.
