Duties And Responsibilities Develop care for client family other involved individuals i.e school personnel probation officer mentor DHHS medical personnel etc Provide therapy case management needed client family caseload Market network community partners current/potential referral sources Examples include but not limited to attending collaborative meetings distributing marketing material providing description services provided Complete all service plans progress notes discharge summaries necessary documentation timely manner Represent Samaritas mission vision community partners Exercise excellence integrity job requirements fulfilled Monitor overall treatment services clients caseload evaluate adequacy meet treatment objectives Participates regularly scheduled staff therapist meetings others deemed necessary SUD Leadership.

Job Qualifications Education Training Licensure/Certification Bachelors Degree Social Work Counseling related field from accredited institution Two years clinical experience working SUD clientele preferred Prior experience interdisciplinary team preferred Licensed Limited license social work counseling Experience Minimum one year SUD counseling experience preferred Minimum Experience working on team preferred Familiarity SUD documentation Knowledge Skills Abilities Ability apply commonsense understanding carry out instructions furnished written oral diagrammatic forms Ability learn utilize technology computer phone apps Maintain confidentiality client employee organization information Willingness work part achieving common goals Constructively communicate supervisor staff Manage multiple functions complex organization Strong organizational skills speak read write fluent English Oral written communication strong prepare letters summaries reports prescribed format conforming rules punctuation grammar diction style h2 substanceusedisorder #casemanager #counseling h2>
