Job summary
The After Care Program assists and supports youth and families navigating post-discharge obstacles.The Aftercare Specialist will work effectively with clients, biological or foster families, staff, the Department of Social Services, and community contacts with a diversity of backgrounds and life situations.
- Travel to and from the residences of youth and families to conduct aftercare casework contacts.
- Exercise strong collaborative efforts with social services and other community providers and agencies.
- Work effectively with biological families as well as foster families.
