    We are seeking individuals to become Home Providers in a meaningful work opportunity within their own home.

    Become a Host Home/Shared Living provider with Mosaic at Home and earn an average annual income of $42,500 (federally tax-free). You must reside in the state of Arizona and own or rent your home.

    About Our Service

    For nearly 40 years, Mosaic has provided improved quality of life, greater stability, and increased community access for people with disabilities through our shared living service, which also offers a satisfying career option for home providers.

    Your Role as a Home Provider

    As a home provider, you will share your home with an individual with a disability. Your responsibility is to maintain a safe and healthy home environment, be a mentor, and assist with daily tasks as needed.

    In this role, you will help cultivate life skills and foster independence. We strive to match interests and lifestyles between people, allowing you to participate in activities together at home and in the community.

    Through our matching process, you can choose the level of support you wish to offer, ranging from minimal support (e.g., budgeting, medication, transportation) to full support (e.g., dressing, toileting, feeding). The income is commensurate with the level of support provided.

    Responsibilities

    • Provide support for daily tasks such as cooking, cleaning, laundry, and budgeting.
    • Aid in skill acquisition and personal growth.
    • Administer medication as needed.
    • Accompany individuals to medical appointments.
    • Support overall health and well-being.
    • Document daily activities via web-based case management application.
    • Plan and participate in fun activities that promote socialization and community inclusion.
    • Comply with all relevant policies and procedures.
    • Maintain open communication with team members.
    • Provide physical support as needed.
    • Offer transportation services.

    Possible Additional Responsibilities

    • Assist with personal care tasks such as bathing, toileting, and feeding.
    • Support medical protocols, such as seizure or fall response.
    • Care for individuals with complex physical or medical needs.
    • Help manage behavioral issues.

    Requirements

    • You must be a resident of the state where you wish to provide services.
    • You must have reliable transportation, a valid driver's license, car insurance, and current vehicle registration.
    • You must have active home/renters insurance.
    • You must own or rent your own home/apartment/condo/townhouse.
    • You must be at least 21 years old.
    • Experience is preferred but not required.
    • You must have reliable computer and internet access.
    • You must complete all required state-mandated trainings.
    • You must maintain an environmentally safe and clean residence.
    • You must pass a State/Federal and APS/CPS background check.
