Key Responsibilities:
+ Have knowledge of the Clients Rights and ensure the client's rights are not violated.
+ Provide services at times requested, including weekend and evening hours.
+ Participate in clinical team meetings to identify client strengths and needs related to recovery plan.
+ Assess clients for housing needs, strengths, and resources.
+ Arrange and refer for services and resources, advocating to obtain entitled services.
+ Coordinate and facilitate housing search and placement, and coach for success.
+ Have a working knowledge of community twelve-step recovery resources.
+ Monitor services by external providers to ensure communication and coordination.
+ Provide experiential training in using self-advocacy skills and resource acquisitions.
+ Provide information and education in accordance with rehabilitation plan and discharge plan.
+ May participate in community events, media and committees as deemed necessary.
+ Function in accordance with organization's policies and procedures.
+ Participate in and contribute to staff development activities.
+ Participate in all required meetings.
+ Other duties as assigned by leadership.
Education and/or Experience Qualifications:
+ Bachelor's Degree from an accredited college or university in social work, psychology, nursing or a closely related field.
+ Equivalent experience may be substituted on the basis of one year for each year of required educational training with a base education of a High School Diploma or GED.
