Job summary
This role operates in a Home and Community-Based Services (HCBS) Setting providing medical social supportive services for Iowans with functional cognitive physical or mental health needs Rather than institutionalizing individuals who have these physical and mental health needs HCBS settings provide these individuals with the opportunity to live receive services in a way honoring to them their abilities
Responsibilities
- Collaborate with local leadership including the HCBS Supervisor HCBS Programmer to develop goals for the individual served This includes Providing input while creating attainable goals objectives for those we serve Creatively problem-solving while holding true to applicable policies practices laws governing bodies Provide training Direct Support Professionals on how best empower people served reach their goals Assisting local leadership provide orientation ongoing training Direct Support Professionals Taking lead learning new effective teaching strategies Looking big picture each individual make sure their goals representative overall needs desires Assisting scheduling appointments individuals served Assuring resources available updated team Provide services based unique goals behavioral care plan Teaching accomplish goal rather completing task Using behavioral support address behaviors success Motivate encourage participate recreational community social activities choose honor choices individual Display understanding apply rights-restrictions only determined interdisciplinary team indicated individual behavior care plan Provide complete consistent accurate documentation progress Provide safe environments instruction help present best selves community Help hygienic needs choosing clothing administering medication ensuring food options suitable dietary needs Be advocate empower control lives possible Treat respect feel empowered live life independently physically possible
