The Reserve at Fountaingrove Memory Care, located in Santa Rosa CA is seeking a dedicated and passionate Caregiver to provide high-quality care to the residents in our community.

Caregiver "Care Partner"
Responsibilities of the Care Partner
- Provide assistance with activities of daily living in a manner that supports our person-centered care philosophy.
- Provides personal care services as outlined in the individual services plans such as assistance with bathing, grooming, dressing, ambulation, positioning, assist to meals, feeding, change bed linens, laundry, etc.
- Assist in daily requirements and tasks as requested.
- Ensure residents are provided privacy, respect, and dignity.
- Follows standard precautions and infection control procedures.
- Completes state required training upon hire and annually.
Required Experience
- Possess, at a minimum, a high school diploma or equivalent.
- Be at least 18 years of age.
- Food Handler Certification, First Aid Certification, Caregiver Certification and CPR Certification.
- Basic computer skills.
- Must be able to speak, read, and write in the English language.
- State Criminal Background Check and LIC 508 Criminal Record Statement
- LIC 503 Health Screening Report
