and various risk assessments utilizing the DSM-V criteria to appropriately diagnose mental health
and/or substance use disorders Develops implements coordinates services,
and monitors evaluates individualized treatment plans Provides individual family,
and/or group psychotherapy sessions utilizing evidenced-based practices Collaborates with
Serenity Health and Wellness Corporation staff community professionals,
and community agencies in coordinating care services maintaining continuity of care Maintains clinical records in compliance with Serenity Health
& Wellness Corporation policies regulatory standards May participate in quality assessment/peer review of documentation Maintains an active professional licensure via Ohio CSWMFT Board Prepares submits accurate timely billing records per Program requirements Complies with documentation requirements outlined program policies procedures for activities e.g.
,assessments treatment plans treatment procedures discharge summaries ,
& preparation required reports Coordinates delivery service provisions acts advocate accessing needed services Identifies provides crisis interventions as needed Maintains familiarity community resources limitations procedures making referrals confidentiality inherent referral process Recognizes need case consultation adhering continued supervision including but not limited to case reviews group supervision individual case reviews teach backs role plays unsuccessful case reviews Maintains contact refencing professionals seeks assistance matters exceed Counselor level expertise scope practice Effectively acts representative agency Manages time organizes work demands appropriately Maintains ability type/use computer programs implemented Agency those maintaining electronic health records Other duties assigned
