- Conduct virtual patient evaluations, including medical history reviews, physical examinations, and pertinent diagnostic assessments.
- Collaborate with the supervising physician to formulate diagnoses & personalized treatment plans.
- Monitor and adjust treatment plans based on patient progress, feedback, and changes in medical status.
- Educate patients about the benefits of lifestyle changes and empower them to take an active role in their health management.
- Provide clear and concise explanations of treatment plans, medications, and lifestyle modifications to patients and their families.
- Maintain accurate and up-to-date EMR records for all patients.
- Collaborate with the medical team to ensure seamless patient care and to continually improve systems, workflows, knowledge base, etc.
- Stay current with advancements in rheumatology and lifestyle medicine through continuous learning, attending conferences, and engaging in professional development activities.
- Maintain all licenses and certifications required for your position & jurisdiction of practice.
- Master's degree in nursing or completion of an accredited Physician Assistant program.
- CALIFORNIA state licensure and/or registration as a Physician's Assistant or Nurse Practitioner, compliant with local regulations.
- Strong interpersonal skills and ability to communicate effectively with patients, families, and the healthcare team.
- Excellent clinical judgment and problem-solving abilities.
- Proficiency in using EMRs and medical software.
- Ability to work in a collaborative, team-oriented environment.
