    Description:
    Per Diem RN HOSPICE-Care Manager for Nursing Homes in the Capital Region. Come experience the rewards of working in end-of-life care


    The Community Hospice has a wonderful opportunity for a Per Diem RN to work as part of our Hospice Care Team caring for patients and families serving Capitol Region Nursing home residents.


    Position Highlights:

    • Case managers are assigned specific nursing homes to minimize travel
    • National pre-taxed mileage reimbursement
    • Meetings and charting conducted via supplied lap top
    • Stipend for cell phone is provided
    • Comprehensive orientation provided

    What you will do:

    • As an RN you will visit patients in the nursing home setting, respond to symptom management needs, and coordinate with our team to provide added support to patients and families.
    • Work days are flexible including weekdays, evenings and weekends.

    What you will need:

    • A current license to practice as a Registered Nurse in the State of New York
    • Associates Degree in Nursing, BSN preferred
    • The RN must be able to communicate effectively, verbally and written
    • 1 year experience as an RN required, preferably in Hospice/Home Care or LTC.
    • Valid Driver's License and Reliable vehicle
    • High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

    Special Per Diem Rates based on monthly hours commitment

    Experienced Pay Range for non-per diem: $35.00-$47.75

    Pay is based on experience, skills, and education.

    Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.

    The pay range may also vary within the stated range based on location.
