Community Health is seeking a licensed Nurse Practitioner to join our Skilled Nursing Facility Service Line that provides primary healthcare to our geriatric population residing in Skilled Nursing Facilities in the greater Rutland Area.
Our SNF Nurse Practitioners perform selective medical services through a collaborative agreement with a supervising MD/DO and must enjoy working with a dedicated team of highly motivated professionals who are committed to providing an integrated, community-based approach to care.
Nursing Home Providers examine and treat patients independently and in collaboration with other health Care Professionals in a nursing home/SNF setting.
They ensure proper illness and injury care and disease prevention, diagnosis, treatment, and recovery.Essential functions include though are not limited to:
Obtains complete medical history and physical examination on patients.
Reviews medical records
Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures
Establishes presumptive diagnosis
Orders appropriate laboratory and diagnostic procedures
Develops a therapeutic plan for patients
Provides orders for drugs and controlled substances.
Serves as the resource go to Provider for designated SNF
Collaborates and coordinates care with SNF Care Coordinators
Works collaboratively with other community partners/Providers to optimize patient care
Assists in the development and monitoring of Quality Improvement for SNF residents
Participates in on call coverage for SNF Service Line
Provides ongoing continuity of care
Knowledge, Education and Skill Requirements
Advance Practice Registered Nurse (APRN), State Licensed with experience in primary healthcare; BLS certified
