    $32.00/Hr + based on experience


    Emphasizes health promotion and disease prevention as they assist patients and families to move rapidly, uneventfully, safely, and as effectively as possible through their episode of care.

    Delegates, oversees and provides quality, safe and developmentally appropriate patient care. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses. Maintains a current Ohio Nursing License.

    Responsibilities


    22Q Nurse General DescriptionProvides nursing care and coordination for patient and families diagnosed with 22Q deletion syndrome or 22Q duplication syndrome.


    Essential Job Functions for the 22Q nurse:

    • Manages new patient intake by identifying resources best suited to treat patient's condition. Gathers all records and data needed prior to 22Q team appointment.
    • Review all 22Q referrals and triage to determine appropriateness for 22Q clinic
    • Trains other nurses to counsel patients and family members regarding 22Q diagnosis, treatment options and appropriate provider triage. Defines requirements for handling patient and family member inquiries
    • Manages multiple patients and oversees care coordination. Collaborates with specialists as needed based on individualized plan of care
    • Coordinates and facilitates feeding clinic flow to ensure patients are evaluated by the appropriate providers based on clinical diagnosis and presentation. Facilitates communication between providers
    • Assist patient in accessing specialized providers through coordinating and facilitating multi-disciplinary visit
    • Serves as the liaison between the providers and families/patients
    • Collaborates with family to discuss updates in plan of care
    • Coordinates with internal and external providers to meet patient's needs
    • Collaborates closely with team coordinator and 22Q NP to help anticipate the needs of the patient population
    • Create and maintain tracking data base for long term care coordination
    • Collaborates with providers to advocate for patient needs or barriers and engage social work or insurance specialist when appropriate
    • Create and disseminate new patient packet information for families in collaboration with the team coordinator
    • Participates in updating helping hands and other education materials
    • Coordinate and participate in the annual 22Q at the zoo event
    • Coordinate and participate in the 22Q family conference that occurs every other year
    • Member of ACPA 22Q SIG
    • Participation in the yearly ACPA conference
    • Submit abstracts and present when accepted serving as a national expert in 22Q nursing
    • Knowledgeable about current research opportunities and QI projects
    Qualifications

    KNOWLEDGE , SKILLS AND ABILITIES REQUIRED


    • EDUCATION: Graduation from an ACEN or CCNE accredited school is required, BSN highly preferred. If AD hired after Jan1, 2012, must successfully complete BSN in 5 years of start date
    • LICENSURE: Licensed as a Registered Nurse in the State of Ohio.
    • Three years pediatric clinical experience or equivalent combination of education/experience preferred.

    OF PATIENTS SERVED:
    Provides safe and competent care for the following groups of patients; neonates, pediatric, adolescents and selected adult patients.


    MINIMUM PHYSICAL REQUIREMENTS


    Must be able to see, hear, stand, walk, speak, read and perform manual tasks with or without accommodation, and care for oneself with little or no difficulty.

    Must be able to independently lift up to 25 lbs.

    Must be able to move or reposition patients of any weight or size with the assistance of another person (s) and/or equipment.
