- Collaborate with healthcare professionals, family members, and caregivers
- Work in various settings such as homes, skilled facilities, and telehealth
- Master's Degree or equivalent experience with NP licensure
- Current RN and NP licensure with prescription authority
- Board certification and relevant CPR certification
- Minimum of 1 year as a Nurse Practitioner
- Strong computer skills and familiarity with medical record software

About the Role:
During visits that last up to one hour, Matrix providers assess a member's health, medical history, medication adherence, social environment, and other risks. This comprehensive health assessment provides valuable insights and helps improve the quality of care, potentially addressing multiple care gaps in one visit.
