    Avera Medical Group Palliative Medicine Yankton seeks a dedicated and compassionate Social Worker to join their team as a Care Coordinator for Palliative Care.


    The ideal candidate will possess strong assessment and planning skills, with the ability to work collaboratively with patients, families, and healthcare teams to provide high-quality care. A Master's degree in Social Work from an accredited program is required, along with licensure as a Social Worker in South Dakota.

    The successful candidate will have experience working with patients facing chronic illness, and be knowledgeable about community resources that support mental health and well-being. They will be able to maintain accurate records and communicate effectively with colleagues and patients.

    This role offers competitive pay, ranging from $28.75 to $43.50 per hour, dependent on experience. The work schedule is PRN as needed.
