    Job Description:
    Promote and market post-acute services to new and existing referral sources within a specified territory and/or account(s). Referral sources may include Acute Care Hospitals, Specialty Hospitals, Physicians and other clinical providers. In conjunction with manager, develop and implement sales plans to achieve established referral, admission and business development targets. Develop, coordinate, and implement educational programs and marketing events to increase awareness of services and products. Identify opportunities for collaboration on new business initiatives, specialty programs and services, with MGB and Non-MGB key stakeholders.

    Work with referral sources to identify patients who could benefit from services, meet regulatory requirements and align with Spaulding and/or MGB Healthcare at Home guidelines.

    Coordinate the referral process to ensure a smooth transition of patients to any post-acute service. Obtain patient assessment information from a variety of sources and transmit data to appropriate systems, primarily through 4next.

    Evaluations may include review of EMR, clinical documentation provided by referral source, meetings/interviews with patients (and/or their family member or significant other) for assessment and patient relations purposes.

    Complete comprehensive preadmission patient assessments when required. Determine appropriate discharge plan, communicate with referral sources and medical providers, and resolve customer concerns with positive outcomes. Participate in patient/family meetings to ensue understanding of requirements for participation, clarify expectations and address potential barriers.


    Key Responsibilities:

    • Patients are evaluated and referred to appropriate post-acute service according to established guidelines and in compliance with local, state and federal regulations.
    • Referral process is coordinated and managed to ensure a smooth transition of patients in accordance with the liaison process and established timeframes.
    • Assist and coordinate in-take and pre-admission screening process.
    • Perform on-site (typically in referring facility) clinical judgement to determine appropriateness of patient for admission.
    • Communicates to patients and families rehabilitation and facility options. Communicates to patients determined appropriate for admission the patient's expectations and requirements for participation.
    • Secures information relating to patients' resources and benefits.
    • Complex face-to-face, in-service education contacts to physicians. And face-to-face, in-service educational contacts to case managers, and other professionals.
    • Performs Market Analysis, market research, and development and implementation of marketing and business plans.
    • Assists in developing the annual market plan for the facility.
    • Continuously monitors the effectiveness have new and existing marketing plans.
    • Assists the various departments of the facility with new business and marketing plans.
    • Represents the facility with business leaders, physicians, the community and the media.
    • Consults other departments, as appropriate, to collaborate on marketing and business plans.
    • Completes all assignments in a timely manner, is thorough and appropriately detailed.
    • Patient assessment information is obtained from a variety of sources and data is transmitted in appropriate systems (4Next/PAS/Epic).
    • Patient evaluations may include meetings and interviews with patient, families or other caregivers as appropriate.
    • Collaborative relationships are developed and maintained with internal staff to ensure customer satisfaction.
    • Referral source relationships are nurtured to ensure and maintain customer satisfaction.
    • Challenging transfers are supported using clinical and supervisory resources.
    • Patients are accepted, confirmed, and qualified as described in the liaison process.
    • Admissions adhere to facility criteria.
    • Pertinent information is communicated to the current medical provider to meet patient and provider needs.
    • Referral influences are identified and communicated to the manager for proper follow up.
    • Discharge plan alternatives are considered to determine the appropriate plan to meet the patients' needs.
    • Payor mix results are achieved according to established targets.
    • Account/territory referral and admissions targets are achieved.
    • Educational information and updates are shared with the referral sources to ensure an understanding of the benefits of post-acute services.

    Qualifications

    Qualifications and Experience:

    • Current license in Massachusetts as RN, PT, OT, SLP (IRF requirement), LTAC, SNF may include LCSW; LICSW, MSW, BSW, LPN, PTA, COTA, RT.
    • Knowledge of Home Care, IRF, LTAC, and SNF levels of care and requirements preferred.
    • Computer literate with working knowledge of Microsoft Office Suite and the ability to learn other software applications.
    • Broad knowledge of various insurances and government reimbursements is desirable.
    • Strong written, verbal and electronic communications skills.
    • Highly developed interpersonal and relationship-building skills, working with a diverse population.
    • Must be able to organize and prioritize effectively while working independently and adapting to changing conditions.
    • Effectively solve problems while meeting demands of all parties.
    • Valid driver's license and clean driving record
    • Formal sales training preferred.
    • Previous marketing/sales experience preferred
