Job Summary
Support the CCI Care Coordination Department in day to day operations and reporting requirements.

The Support Specialist has a basic understanding of Care Management processes to support and intervene in areas allowable under the scope of practice for this role.

Ability and willingness to develop expertise in administrative and technical support related to the operational functions of the Care Coordination Department.

Awareness of health care regulations around transitions of care and care management. Effective communication skills to engage collaboratively with care management staff and all other health care professionals.
