Job summary
We are seeking a licensed vocational nurse (LVN) to provide skilled nursing care to patients in their homes under the supervision of a registered nurse (RN). As an LVN with Remedy Home Health Care you will deliver hands-on skilled nursing care based on individualized care plans.- Provide skilled patient-centered nursing care based on individualized care plans
- Accurately document all visits assessments and treatments using electronic health records (EHR)
- Communicate effectively with patients families and the home health care team
