- You will administer assessment tools and gather data for assessment, and to assess for optional needed services
- You will interview families to determine social work needs of the participant
- You will contact community resources to coordinate services
- You will provide initial and on-going assessment of participant's social, emotional and cognitive status and needs, involving participant and other staff members in the assessment
- You will ensure accurate documentation is maintained for all assessments, treatment plans, and evaluations
- Master's degree in Social Work required
- State social work licensure preferred
- Minimum one (1) year experience at Master's Level working with an Interdisciplinary Team, preferably in a post-acute setting
- Experience with geriatrics and disabled adults preferred
- Utilization Review experience preferred

The PACE MSW is responsible for assessing the psychosocial wellbeing and safety needs of PACE members. The MSW is a member of the Interdisciplinary Team that is responsible for determining a coordinating plan of care for each participant and implementing that plan of care. The MSW is engaged in service delivery and service determination for the PACE program, an all-inclusive health plan for seniors. The Program of All-Inclusive Care for the Elderly (PACE) is a comprehensive health plan for those who require long term care services (like caregiving) and are living in the community. PACE members are served by an interdisciplinary team focused on collaborating with members on a plan of care that helps them stay living in the community. The PACE philosophy is centered on the belief that it is better for frail individuals and their families to be served in the community whenever possible. Position Summary
The PACE Social Worker MSW is an excellent opportunity at MultiCare, responsible for aiding participants and their families with personal and environmental difficulties that predispose illness or interfere with remaining in their home environment. In this role you will be able to apply your strong analysis, interpretation and interpersonal skills to support patients through complex issues.
