The Central Intake Licensed Practical Nurse, LPN registers and schedules all Home Care patients/clients. This role coordinates the intake process with physician offices, emergency departments, and other healthcare facilities and ancillary areas. They assist with insurance verification and related support duties. Welcomes patients/clients by offering service, assistance, and determining the reason for their service to facilitate activities to meet their needsWorks collaboratively with healthcare facilities and physician offices in support of the admission process
Performs system maintenance and clerical duties, including merging patient information when two accounts exist, loading new doctor and facility information into system.

Work Experience1 year of clinical patient care experience (e.g., PCT, CNA, LPN) in an acute care setting, long-term care facility, or physician practice (preferred)
Training

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.
