Pharmacist Required Duties:

• Conduct patient assessments to determine appropriate medication therapy
• Collaborate with healthcare professionals to develop comprehensive treatment plans
• Provide medication counseling and education to patients
• Monitor patients' response to medication therapy and adjust treatment plans as needed
• Utilize utilization management techniques to optimize medication use and control costs
• Ensure compliance with all relevant laws, regulations, and standards
• Maintain accurate patient records and documentation
• Participate in pharmacy quality improvement initiatives
• Adhere to aseptic technique when preparing sterile products

Pharmacist Required Experience:

• Doctor of Pharmacy (Pharm.

D.) degree from an accredited college of pharmacy

• Current state pharmacist license in good standing
• Minimum of 2 years of experience in a clinical pharmacy setting, preferably in pain management or critical care
• Strong knowledge of acute pain management principles and practices
• Proficiency in utilizing pharmacy software systems for medication order entry and clinical documentation
• Excellent communication and interpersonal skills
• Ability to work effectively as part of a multidisciplinary healthcare team
