Summary
This Medical Social Worker position is in Social Work Service Line at the VA Maine Healthcare Systems, located at the Togus location. This position is a full-time at 40 hours per week.Duties
Social Work :Medical Social Work services as needed to inpatient Veterans on the Medical-Surgical and SCU units, Veterans treated in Ambulatory Day Surgery, and Veterans receiving specialty services from outpatient Specialty clinics such as Oncology, Gastroenterology, Cardiology, etc. These services include the provision of high-quality case management, supportive counseling, advocacy, discharge planning and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the Veteran. Collaboration and consultation with other members of the interdisciplinary treatment team is an important aspect of this position.
Major duties include but are not limited to:
