    Description

    Job Summary: The School Based Health Coordinator will work in collaboration with a Lead School Liaison and a Leadership Team to assess, plan and develop an Expanded School Based Health Program. Program will include coordination and linkage to universal prevention programming, early intervention and intensive treatment by using evidence-based practices and programs. Activities will include suicide prevention, anti-bullying, drug and alcohol prevention, academic skill builders, mentoring, pro-social skills, link to further assessment/evaluation and individual/family/group therapy.

    Duties and Responsibilities:
    • Lead the assessment of prioritized school needs in collaboration with Lead School Liaison.
    • Assist with development of a Leadership Team.
    • Assist with coordination of necessary training.
    • Assure implementation of evidenced based practices and programming.
    • Work with Lead School Liaison and Leadership Team to implement project goals and objectives to develop universal prevention programming, early intervention and intensive treatment.
    • Work in collaboration with local stakeholders that will assist with project implementation.
    • Work with School Improvement Counsel and Student Assistance Teams as well as school faculty.
    • Participate in the development of system to collect and track necessary data.
    • Assist to create a welcoming environment for families and link families to necessary support.
    • Lead required evaluation of project.
    • Attend necessary trainings and required meetings.
    • Adhere to grant and fiscal requirements.
    • Complete all necessary paperwork.
    • Perform other duties as assigned.
    Required Skills/Abilities:
    • Knowledge and experience of youth and adults with mental health diagnosis.
    • Proficient creation of curriculums and model programs.
    • Ability to communicate effectively both verbally and written.
    • Experience with collaborative program development.
    • Knowledge of community resources.
    • Ability to work under pressure and multitask.
    • Proficient and accurate documentation.
    • Ability to use Adobe and Microsoft software.
    Education and Experience:

    Requires Associate Degree in Social Work, Counseling, Psychology or related human services field. Prefers 1-2 years of experience working with at risk youth. Prefers experience with mental health and substance abuse prevention and intervention. Must have valid driver's license and proof of automobile insurance as required by law.
