Job Summary
The Clinical Advocate's primary responsibility is to assist with population management and development of advanced medical home model in primary care offices.The advocate will communicate policies and protocols and workflow changes to primary care offices.
This includes monitoring population management reports, pre-visit planning, and consistent office communication in the form of huddles and monthly office meetings.
