    Description

    Join us:

    • Position: LPC, LCSW, LMFT, or RN for Superior Health Plan STAR + Plus
    • Schedule: Monday - Friday, 8 am - 5 pm (CST)

    In this role, you will handle care management duties for adult members dealing with serious and persistent mental illness, ensuring quality and cost-effective care across the care continuum. You'll also be responsible for crisis call management, assessing member risks, and coordinating additional support when needed.

    Key Responsibilities:

    • Assess, plan, and coordinate all aspects of physical and behavioral health services
    • Develop personalized care plans based on assessment
    • Coordinate services among healthcare providers and non-clinical staff
    • Monitor referrals to support member's overall care plan
    • Travel for field visits as required

    Requirements:

    • Master's degree in relevant field or RN with psychiatric experience
    • Minimum of 3 years of experience in direct behavioral health/psychiatric care
    • Valid licensure (LCSW, LPC, LMFT, RN)
    • Experience in community mental health or social services
    • Ability to work independently with minimal supervision
