In conjunction with the Wound RN, conducts ongoing reassessment and gathers data relevant to the individual needs of the patient or family.

Performs admission and ongoing focused system measurements, as appropriate to specialty; obtains patient history, including objective and subjective data.

Observes patient's clinical condition; reviews diagnostic information; recognizes implications of abnormal diagnostic results; identifies problems typically associated with a diagnosis or treatment; reports pertinent information to appropriate personnel for further direction.

Summary
Position Title: LPN – Treatment Team (PRN)
Hours: 5:00 a.m. – 1:30 p.m. 8-hour, PRN
Position Summary
Under the supervision of a Wound Care RN, the Licensed Practical Nurse (LPN) – Treatment Team utilizes a general understanding of the principles of nursing and basic physical assessment skills in the development and implementation of individualized nursing care plans to ensure that the needs of the residents are met. The LPN assists in the orientation and supervision of nursing personnel, attends to the daily operations of the unit on a per-shift and unit level, and assumes responsibilities of a leadership role as needed.
