    Position Summary

    To participate as part of the Interdisciplinary team, through implementing the Individual Specific Plan and providing feedback on the individual's progress. To be an advocate for the individual and ensure all health and safety needs are met with dignity.

    Position Duties and Responsibilities

    1. Contribute to the evaluation of the individual's progress and provide input to the team with regard to the development of appropriate plans to achieve specific ISP outcomes.
    2. Complete written and verbal reports/ documentation related to daily progress.
    3. Act as a positive role model to individuals served.
    4. Provide community inclusion support and behavior management.
    5. Assist with hygiene, grooming, medication delivery, and any other areas identified within the ISP.
    6. Provide transportation (using personal vehicle) the individual served to access the community.
    7. Identify social, volunteer, economical, recreational, and educational services in the community that will meet the individual's needs in addition to pre-planned activities.
    8. All activities related to employment goals will be identified as priorities.
    9. Provide emotional support and feedback to the individual and their family.
    10. Complete all required training and work within all HeartWell Policies & Procedures.
    Requirements:

    Position Requirements

    • Minimum – High school diploma or equivalent
    • Current and Valid Driver's License
    • Safe insured/ registered vehicle
    • Must be 18 yrs or older
    • Bilingual preferred
