    Job responsibilities:

    • Connect patients with community resources and support services as appropriate to identified needs.
    • Document assessments, plans, and communications in the medical record.
    • Work 40 hours per week with 8-hour shifts.
    • Support patient and family meetings related to care planning and transitions as needed.
