    **Join Our Dynamic Team as a Physical Therapy Assistant**

    **Job Description:**

    - Partner with residents and care providers to deliver exceptional physical therapy services.
    - Support and promote our mission and values, enhancing residents' independence and functional abilities.
    - Perform therapeutic procedures under the guidance of a physician or other healthcare providers, ensuring the utmost safety and care.
    - Develop individualized therapeutic plans, integrating input from residents and primary caregivers to reach functional goals.
    - Communicate effectively with your team, bringing any concerns or status changes to your supervising therapist.
    - Participate actively in team meetings to contribute to collaborative care.
    - Monitor treatment effectiveness and adjust programs as necessary to meet established goals.
    - Ensure thorough and accessible documentation of all therapeutic interactions.
    - Recommend and educate residents on the use of assistive devices, providing clear instructions.
    - Stay compliant with all healthcare regulations, including Medicare and Medicaid.
    - Engage in the development of innovative programs and services in Rehabilitation.
    - Pursue ongoing learning and professional development opportunities.
    - Maintain a clean, organized work environment while adhering to infection control procedures.
    - Keep your required licenses, certifications, and competencies up to date.

    **What We're Looking For:**

    - Associate degree in a related field.
    - Valid PT Assistant License.
    - Basic Life Support (BLS) certification.
    - Preferred prior experience in a long-term care or skilled nursing facility.
    - Strong verbal, written, and presentation skills.
    - A responsible, supportive team player with excellent interpersonal and customer service abilities.
    - Analytical mindset with solid decision-making skills.
    - Proficient in Microsoft Office and capable of managing time efficiently.
    - Compassionate, resident-centered care approach.
    - Ability to communicate effectively in English.

    **Working Conditions/Physical Demands:**

    - Must possess the physical and mental capabilities to perform tasks that involve a combination of:
    - Sitting
    - Standing
    - Walking
    - Bending
    - Stooping
    - Twisting
    - Reaching
    - Ability to lift up to 50 pounds; above 50 pounds requires assistance.
    - Frequent lifting and carrying of up to 20 pounds.
