The salary for this position is:
$48.00/Hour


KEY ESSENTIAL FUNCTIONS:
+ Serve as a leader in the delivery integrated nursing services to the clinic clients in accordance to health indicators.+ Provide integrated peer care services to the client population at the clinic as indicated by the OMH, CCBHC expansion scope of practice+ Provide clients with services in the filed as required+ Participate in grant related workshops and trainings+ Use the grant portals; SPARS to enter any grant required data in terms of service provision, clinical indicators+ Provide health assessments and medication management services for children, adolescents, and adults diagnosed with severe and persistent mental illness.+ Complete pharmacy reviews.+ Will work closely with the clinical staff on following up on prescribed medication, care coordination with pharmacy. + Will record patient"s vital signs and obtain patient history by integrating the health screening, PSYCKES clinical record as part of the triage process and assisting patients with completion. + Perform health assessments and monitor all high indicators.+ Provide crisis intervention, including emergency hospital admission support.+ Coordinate medical/medication services with external providers including clinics, doctors, pharmacies, health homes.+ Will enhance patient"s health and self- management skills as developing wellness goals in the use of EBPs on (WSM, Tobacco Cessation, nutrition, exercise and recovery and wellness) and maintain record of these activities. + Administer psychotropic medication treatment, including administration of intravenous medications.+ Work as a member of a multidisciplinary treatment team and practice client centered and holistic care.+ Maintain compliant and timely documentation of services, case trackers and related clinical contacts in accordance with OMH regulations and organizational expectations.+ Perform clinical, compliance, and administrative duties as instructed.+ Participate in QA, UR, and IRC meetings.+ Carry out all relevant policies and procedures of the agency issued by management in accordance with funding agency guidelines. This does not limit the assignments of duties or exclude the performance of other duties, not mentioned as assigned by the supervisor.


REQUIREMENTS:
+ Previous relevant experience in a Mental or Behavioral Health environment required.+ NYS Registered Nurse License required.+ Ability to use business acumen to exercise good judgment and decision-making.+ Ability to deal with sensitive issues with diplomacy and discretion.+ Must be able to multi-task with strong organizational skills.+ Excellent interpersonal skills and able to communicate both verbally and in written form.+ Commitment to cultural diversity and sensitivity.+ Ability to maintain a professional demeanor under pressure, and operate with a keen sense of urgency and commitment to quality.+ Bi-lingual (Spanish/English) preferred+ Excellent computer skills including, Microsoft Windows, Excel, PowerPoint, and electronic communications tools: internet and email and Excellent time management skills.*Acacia Network is an equal opportunity employer*Job Requirements
