    Moosehaven is a private membership retirement community located on the banks of the St Johns River, in the heart of Orange Park, Florida.     We are currently looking for Full- time and Part time Registered Nurses for the following shifts:
    • 1st Shift 6:45 am -3:15pm
    • 2nd shift 2:45 pm -11:15 pm
    • 3rd shift 10:45 pm- 7:15 am

    GENERAL SUMMARY:

    The Registered Nurse is responsible to the Department Director, and is to be clinically competent; delivers care to residents utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with resident, significant others, and other health team members while maintaining standards of professional nursing.

    ESSENTIAL JOB FUNCTIONS:

    • Utilize the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the resident's care, as evidenced by client/staff observation.
    • Assess the resident; collect and document the resident's current health status and medical history. Complete and/or review Admission Data Record as defined by policy. Make accurate and ongoing assessment of resident's status and respond appropriately.
    • Plan/Implement; set priorities for nursing action in a logical sequence according to resident's needs and formulate a plan of care by the appropriate selection of Nurse Practice Standards. (UOC's) The Plan of Care is initiated on resident admission based on all aspects of the initial assessment. Plan includes physiological, psychosocial, and environmental factors: resident/significant other education and resident discharge planning.
    • Evaluate; perform ongoing assessment and revise plan of care based on new data and resident's condition.
    • Document accurate and ongoing assessment of resident status. Document resident care which reflects nursing intervention, resident response to care provided, resident needs, problems, capabilities, limitations, and progress toward goals. Documentation includes evidence of appropriate resident/significant other teaching, and the understanding of these instructions is noted in the medical record.
    • Communicate with the physician regarding resident needs, the nursing assessments, and recommendations on a daily basis, and as needed, as evidenced by client feedback and Operations Coordinator observation.
    • Collaborate with other nurses and health team members to ensure the resident's well being as evidenced by staff/client feedback.
    • Teach resident/significant other appropriate health information in a timely manner and share written informational material as evidenced by client feedback and Operations Coordinator observation.

    MINIMUM REQUIREMENTS:

    • Graduate of an accredited school of Nursing
    • Valid registration as a Registered Nurse in the State of Florida.
    • Previous experience in patient care setting preferred.
    • Knowledgeable regarding the nursing process and its application, including the

    assessment, planning, implementation, and evaluation of nursing care.
