    Care Connections is recruiting for a 0.9 FTE Extensivist, to serve patients in our clinic. Care Connections is a small, closed panel practice, serving patients with chronic medical conditions. Position is dayshift, Monday-Friday, with on-call rotation. Holidays follow LGHP schedule. Duties include both outpatient office visits/primary care, as well as inpatient hospital rounding on paneled patients. Provide high level and patient-centered care in the discipline of Extensivist Medicine.
    Provide ambulatory, home, inpatient and consultative nursing home care to patients in the High Risk Medical Pilot
    Attend to patients in the High Risk Medical Pilot office: Care Connections
    Attend to patients enrolled in the program in the hospital on a daily basis in a consultative fashion
    Take night call in rotation with other medical staff to provide 24/7 coverage for patients via phone.

    Coordinate work with nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other health care providers.
    Recommends and orders tests to determine extent of illness or help diagnose condition.

    Prescribe or administer treatment, therapy, medication, vaccinations, and other medical care to treat or prevent illness, disease, or injury.

    Refers patients to medical specialists when necessary.
    provide medical care plan consultation during meetings.
    Provides clinical and didactic teaching to residents, medical students and staff on related topics and issues.
    Monitor financial performance of the practice as well as utilization of all patients enrolled thru the practice.

    Represents the practice at Hospital/LGHP committees in such a manner that the burden of attending the meetings is reasonably distributed among all of the physicians in the group.

    Assists in recruiting and interviewing of new physicians and staff to the practice.
    A Medical Doctor (M.Completion of Internal Medicine Residency or Family Medicine Residency.
    Board Eligible or Board Certified in Internal/Family Medicine.
    Current medical licensure issued by PA State Board of Medicine or the PA State Board of Osteopathic Medicine.

    A health care provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e.

    not excluded from participation in Medicare, Medicaid or any other federal or state health insurance program.

    At least three years clinical patient care experience as a general internal or family physician with acute care, hospitalist experience.
