- Master's Degree in Human/Social Services with one (1) year (two (2) preferred) OR Bachelor's Degree in Human/Social Services with three (3) years (four (4) preferred), of experience in specialized behavioral health services of specific mental health illnesses of assigned population.

The ability to understand and grasp the principles and practices of providing quality services, in fidelity with a person centered behavioral health care environment, is critical to success in this position. Commandment of effective oral and written communication skills, excellent listening, interpersonal and recovery-oriented interpersonal skills, and a high, detailed level of computer literacy and faultless accuracy in maintaining and managing sensitive data is essential. Must be able to work as a highly functioning member of a team, maintain a high level of confidentiality and be able to work effectively with all levels of the organization.
- Ability to manage multiple electronic health records, databases and maintain tracking of data through spreadsheets to oversee a person's care. Responsible for thorough communication with multiple sources, as well as utilize engagement tools and interventions to provide outreach to those we serve.

RESPONSIBILITIES/REQUIREMENT
As a member of the Adult Ambulatory Team, this professional level position consists primarily of duties requiring advanced knowledge of mental health and substance used disorders and their effects, which are predominately intellectual, and is designed to exercise discretion and independent judgment in providing external and internal outreach services, to targeted populations, for those who are either already part of the Pittsburgh Mercy (PM) family of services or for those entering into the PM family of services. Working closely with the appointed ambulatory team (which are teams assigned with providing outpatient treatment and multi-disciplinary services to adults with severe mental health illnesses and/or substance used disorders, and whose mental health diagnosis meets the dimensions for outpatient and/or partial hospitalization treatment criteria), as a learned mental health professional, this position will also work with the Practice Managers, Clinical Leads, Clinicians, Assessors, and other medical staff, in providing care management and coordination of services for individuals served on a specific ambulatory outpatient team.
The Behavioral Health Engagement Care Manager must be able to act and respond appropriately and independently, using obtained experience and education, in sometimes unpredictable situations, in managing all aspects of the client's care and will champion their duties using Pittsburgh Mercy's philosophy, values, policies, procedures and standards. In addition to maintaining departmental policies, procedures and goals, the above-stated job functions are to be performed in accordance with all applicable laws and regulations.
