    CHCW has an opening for a full time Community Outreach Liaison to join our team. You will be responsible for helping patients and their families to navigate and access community services, other resources, and to adopt healthy behaviors. You will support our providers, case managers and RN care coordinators through an integrated approach to care management and community outreach. This position offers the flexibility of a hybrid remote/on-site work schedule.

    Qualifications include:
    • 2 year degree in related field or equivalent combination of education and experience is required.
    • Prior experience working in a health care environment, preferably a community health setting is strongly desired.
    • Experience working with Health Benefit Exchange to assist community members with enrolling into a health plan is preferred.
    • Patient Assister Certification is required (may be obtained after hire).
    • Valid driver's license and auto insurance is required.
    • Bilingual Spanish/English is required.
    • Pre-employment drug screen required.
