    This clinical position oversees Tiburcio Vasquez's Health Centers (TVHC) Population Health Department, whose mission is to improve the health of our patients and our community by addressing disparities through health education, patient empowerment, and advocacy.

    Directly reporting to the CMO, and as a member of the management team, TVHC's Medical Director of Population Health (MDPH) provides leadership, motivation and direction across TVHC to ensure that the organization supports best population health practices.

    The MDPH provides guidance to Program Directors & Managers of our Population Health programs.

    The MDPH oversees successful integration of Population Health programs, monitors and grows strategic relationships within the community and other organizations in order to ensure both healthy patients and healthy communities.

    The MDPH believes in an approach that aims to improve the health of the population, as well as acknowledging social determinants which have a strong influence on health.

    TVHC Population Health programs are expansive.

    Our programs conduct outreach to community members who may not have a medical home, our programs seek to identify and address barriers to care, and they provide preventative health education to patients and in the community.

    The SMDPH understands the value in tracking outcomes to assess effectiveness.

    The position provides guidance among existing programs in order to match patient and community needs with the actual scope of services being provided and looks for opportunities for expansion.

    The position also works collaboratively with the areas of primary medical and dental care for the purpose of decreasing barriers to care.


    Compensation:
    $195, $241,467.20, depending on experience


    Responsibilities:

    Provides at least sixteen (16) hours per week of direct clinical medical services in the area of board certified (or board eligible) medical specialty in accordance with the highest applicable standards of medical and professional practice.

    Responsible for understanding the TVHC's Population health programs and overseeing respective population health program directors and/or managers. Identifies practice improvement opportunities.

    Develops and monitors integration of new and existing TVHC population programs with TVHC clinical programs to ensure full patient engagement and health outcomes.

    Works between various service sites to coordinate integration Makes a positive impact on resource utilization. Regular data analysis as a means for process improvement, informed decision-making, and program implementation.

    As a member of senior management, works closely with the CMO to ensure TVHC's goal of effective health and wellness of our patients and community is met.

    With Population Health directors, responsible for program budget allocation and staffing.

    Provide guidance on how TVHC can develop and implement solutions that advance continuity of care and help manage costs Emphasizes partnerships, prevention and health education, through a range of health promotion and research activities Ensures protection and promotion of health and improvement in people's capacity to participate in society by aiming to prevent illness, injury, disability and early mortality across the entire population; with a focus on populations at greater risk of poor health.

    Provides program directors with information and guidance for bringing service lines together for the improvement of integrated care regardless of patient point-of-entry.

    Keeps Population Health Directors and Managers accountable for the performance of their individual programs. Conducts performance reviews for direct reports in conference with the Chief Medical Officer.

    Serves as advisor for Population Health directors as they manage their grants and/or funding, develop job descriptions and establish program objectives.

    Upon the recommendation of the CMO, may assist the Program Director with the negotiation and representation of grants to funders and external stakeholders.

    Works to map internal referral processes, reviewing existing forms and methods of documentation and approval. Helps identify patient social determinants of health in order to identify barriers to improving patients' health. Works with Program Directors and Administrative staff to recommend improvements in referral and data gathering processes. Works with CMO and Information Technology department to gain understanding of Electronic Health Record systems in use throughout TVHC. Works closely with other departments including, but limited to, medical, dental, operations, QI, and behavioral health.

    Ensures confidentiality regarding privileged administrative and client information in a professional manner Attends clinic and external meetings according to work priorities as assigned.

    RequirementsMD or DO with board certification in designated primary care specialty field of practice. Minimum three years of experience in administration and supervising professional level employees preferred. Minimum of five years experience in healthcare program development and implementation experience preferred. Experience with implementing start-up projects successfully. Previous experience working with culturally diverse adult populations.


    Qualifications:
    Proven ability to effectively lead programs and manage professional-level employees. Commitment to the vision and philosophy of Population health. Operational and programmatic experience preferred. Ability to use any applicable clinical background for staff education, program implementation, and development of workflows as appropriate. Experience with systems integration. Able to communicate effectively, both verbally and written. Excellent interpersonal communication skills; proven ability to problem-solve and be solutions-oriented. Excellent organizational and administrative skills. Strong ability to work as a team member with fellow co-workers, supervisor and agency staff. Culturally sensitive and understanding of underserved patient populations. Demonstrate flexibility regarding job duties and assignments. Ability to travel to designated locations as required by supervisor.
