- Provides inpatient consultation for diabetes management and evaluation in collaboration with the primary care nurse.
- Collaborates with Case Management on continuity of care related to glycemic management.
- Documents assessment data and clinical recommendations grounded in evidence based practice.
- Assists with coordination of diabetes education for all clinical staff, including physicians.
- Registered Nurse, State of Florida
- Associate's degree required.
- Experience with current diabetes treatments and management with inpatient populations
- Valid license as a Registered Nurse in the State of Florida required.
- Certified Diabetes Care and Educator Specialist (CDCES) or Board Certified - Advanced Diabetes Management (BC-ADM) required. Must provide proof of CDCES certification or ability to obtain in 6 months.
- Must have current AHA BLS certification

The role you'll contribute:
The role of the Diabetes Education Specialist is to perform professional consultation and educational services for persons with diabetes.
The program will include developing and implementing prevention strategies as well as providing education to diabetic clients and their families.
Responsible for the curriculum development for courses offered to nurses, patients and their families.Train and educate nursing staff in diabetes and health clinical procedures; coordinates in-service training, continuing education, and other related diabetes and health clinical training.
Responsible to monitor the glycemic outcomes for the organization and develop appropriate action plans.The value you'll bring to the team:
