- Evaluates and treats center patients in accordance with standards of care.
- Assists in the coordination of patient services, including but not limited to specialty referrals when appropriate, cardiology subspecialists and vascular surgeons, etc.
- Acts as an active participant and key source of medical expertise with the care team.
- Partners with the primary care physicians and nurse practitioners in a tone of cooperation in practice by displaying a professional and approachable demeanor
- Completes all medical record documentation daily working with a quality- based coder to optimize comprehensive documentation.
- Follows policy and protocol defined by Clinical Leadership
- Meets with primary care physicians (including nurse practitioners) and market medical director and about quality of care, review of outcome data, and medical records issues.
- Clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care
- Support our longitudinal care model for heart failure with focus on disease management and prevention.
- Support readmission reduction initiatives for heart failure and CAD.
- Create a practical framework for upstream stroke prevention.
- Help coordinate care for CKD stage 5 patients needing AV graft placement prior to initiating hemodialysis.
- Interpret and document in detail test results to determine how effectively the heart and or cardiovascular system is functioning i.e., Echocardiograms, stress test, etc.
- Coordinate with PCP
- Coordinate patient care with cardiology subspecialist i.e., interventional, EP, Thoracic, etc.
- Graduate of accredited MD or DO program of accredited university.
- Current and unrestricted medical license or willing to obtain a medical license in state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
