    Assists with planning, implementing, evaluating and revising the plan of care in collaboration with clinical nurses and other disciplines to provide optimal care for patients/families.

    Serves as a consultant to provide information and expertise to healthcare providers at all levels related to the needs of wound/ostomy patients and their families.

    Serves as a resource to clinical staff regarding evidence-based pressure ulcer prevention strategies; Consults with staff on appropriate use of specialty beds; Coaches staff on wound assessment and wound management techniques.

    Assesses and recommends topical management of wounds for nurses and physicians (i.e., pressure ulcers, vascular and diabetic lower extremity, traumatic and vacuum-assisted closure (VAC) wounds); Assists clinical nurses with problem solving and competency development with simple wound VAC therapy.

    Applies complex wound VACs.

    Assesses patients for compression leg therapy per MD request and applies when appropriate; Orders equipment and supplies as needed for care.

    Instructs staff in use of equipment as necessary. Assists with discharge planning in collaboration with clinical nurse, case manager and social service to ensure continuity of care. Provides preop education/counseling to help the patient/family cope with emotional impact of ostomy surgery.

    Interprets surgical procedures to the patient/family as indicated by physician; Interviews the patient, reviewing history to set priorities in post-op care and management.

    Selects and marks the stoma site. Introduces patient to products and appliances; Selects and applies post-operative collection device. Instructs patient in self-care to include family and/or significant other. Provides for return demonstrations before discharge; Coaches staff on how to use stomal appliances and teach patient about self-care.

    Evaluates stomal condition and care by daily visits, reporting unusual conditions to the clinical nurse and/or physician; After discharge, assesses change in stomal size, skin problems, and changes in body weight, pregnancy or any other condition requiring a change in management technique during outpatient visit.

    Provides counseling and/or referrals to patient and family as needed. Participates in orientation programs to keep nursing staff and other hospital personnel abreast of current evidence-based clinical practice.

    Participates in Skin Care Team as the clinical expert to expand knowledge of clinical nurses and improve skin/wound assessment, prevention and care across the system.

    Tallies statistics and records of procedures and treatments of wound/ostomy caseload; Engages in quality assessment/process improvement and research activities related to skin, wound, and ostomy care to promote evidence-based nursing practice.

    These activities include evaluating new products related to skin, wound, and ostomy care. Identifies performance improvement opportunities; leads the PI process for projects related to wound and ostomy care.

    Background/Expertise:
    Requires BSN or non-nursing Bachelor's Degree with MSN; ADN graduate with less than 5 years of acute care experience required to sign agreement to complete BSN or MSN within 3 years of hire


    Note:

    Employees hired as an ADN prior to October 2011 are not required to obtain BSN or MSN.BLS issued by the American Heart Association required.

    Minimum of three (3) years experience in general surgical nursing required.
    Membership in Wound, Ostomy and Continence Nurses' Society recommended.

    Current license to practice as a Registered Nurse in the State of Oregon; Advanced educational preparation in wound and ostomy management from an institution accredited by Wound Ostomy Continence Society; CWOCN certification is preferred.
