    Job Summary:
    Support Coordinator is responsible for providing support coordination to individuals of all ages with

    intellectual/developmental

    disabilities.

    This includes assessing service needs, developing the NJISP (Service Plan) for access to services; liaison with client, families, programs and service providers, ongoing monitoring of clients' service needs, advocacy; consultation and education to client, families and community and completing Monthly Tools.

    Requirements:
    Bachelor's degree in public health or related field, Master's degree preferred.
    Excellent oral and written communication and presentation skills.
    Professional demeanor and appearance.
    Proficient skills to promote excellent client relations and customer service skills.
    People Management skills and the ability to network and manage a team.
    Excellent organization and time management skills.
    Must have a valid driver's license and personal vehicle-driving to client homes and other locations are required.
    Completion of ALL New Jersey DDD/CDS required trainings or willing to complete trainings.


    Responsibilities/ Essential Job Function:
    Employee must be able to perform the following essential Job Function- with or without reasonable accommodations.

    Assess and complete a Person-Centered Planning Tool [PCPT] and information gathering packet with individual, their family and team to gather information and identify goals, eligibility, outcomes and
    services needed. Using NJCAT to Identify Support Needs

    Coordinate and facilitate team meetings off-site at either Residential and/or Day Treatment Programs, to identify service needs and develop individual support plans [NJISP].

    Coordinate with individuals, their families, and mentors to identify outcomes and utilize their budgets to achieve those outcomes.

    Ensure that the service plan addresses personal, medical, nutritional, health, safety and supports, and employment issues, including all assurances that are required by Medicaid.

    Assisting in identifying services and supports that will achieve that individuals stated outcomes and can be accessed from a provider who is appropriately qualified by DDD to provide those services.

    Drafting the NJISP (Service Plan) and making revisions with the appropriate input from all member of the team as needed to be sent for DDD State Review for approval.

    Assisting individuals and their families, in assuring that the services they have identified will meet desired outcomes.
    Connecting individuals and families with generic resources and natural supports.

    Entering all data into the electronic record (iRecord) and Coordinate with DDD staff to approve the NJISP (Service Plan), authorize the service and provide quality assurance.

    Maintain complete and current individual records and other required reports, service reporting and Documentation in compliance with Agency, State and Federal standards.

    Provide quality case management and maintain phone and face to face contact with the individual (Monthly Tools), their support team and provider to ensure that the NJISP and services are being delivered appropriately and within a time efficient manner.

    Participate in all meetings, in-service education, and trainings as required by DDD and the agency.
    Assist with the performance review process.
    Assist with the recruitment and interview process.
    Create new employee personnel files and filing documents into appropriate employee files.
    Update database records and process paperwork for new hires, terminations and other status
    Participate in on-call schedule.

    Perform other duties as assigned by the Support Coordinator Supervisor and/or Administrator in keeping with general requirements of the position.
