OUTPATIENT, EMPLOYED POSITIONS
Sites include: Edgewater, Orange City( 3 12 hours shifts here), Ormond Beach, Palm Coast and South Daytona
* Work Hours: To work or be available to provide medical coverage during standard office hours, Monday through Friday, from 7:00 a.m. to 6:00 p.m.

Physicians:
Physician shall receive compensation per annum to be reconciled against the Productivity Incentive Compensation as set forth below.
● Thirty-Five Percent (35%) of Net Collected Receipts up to $400,000 per year;
● Forty Percent (40%) of Net Collected Receipts between $400,000 and $600,000 per year; and
● Physician may be eligible to participate in the following revenue pools, subject to a mutual agreement between client administration and provider.
● Mid-Level Supervision Revenues Pool
● Ancillary Revenues Pool
● Research Revenues Pool
● Value Based Reimbursement Pool
● Physician Recruitment Bonus
● TO BETTER UNDERSTAND INCOME, SEE INFO BELOW:
● Forty-Five Percent (45%) of Net Collected Receipts in excess of $600,000 per year. Personally Performed - $500,000 Net Collections à $180,000
○ 35% of $400,000 = $140,000
○ 40% of $100,000 = $40,000
● Ancillary Pool
○ SRT, Ultrasound, X-Ray, Lab, etc.
○ You would receive a portion based on similar percentage of collections you bring into the practice, in order to be compliant with Stark Law.
● Value Based Reimbursement Pool
○ $40 per Medicare Patient
○ 250 Patients to Begin
○ Other Quality, Shared Savings, and Misc. Payer Performance Reimbursement Programs Negotiated by client.
● Mid-Level Supervision Pool
○ Receive a percentage of their collections, starting Q1 2021.
● Research Pool
○ Depending on the level of involvement of the provider.
● Recruitment Bonus
○ $5,000 per physician
●
● Maximum of twenty (20) days for personal leave,
● Five (5) days for continuing medical education per annum, and
No more than ten (10) days shall be taken consecutively
