**Family Caregiver Support Program is a service available to unpaid caregivers of adults who need care in their homes. Staff provide practical information and connect the caregiver to local free or low-cost resources/services meeting their needs. In this position, you will provide case management services to unpaid family caregivers, including information and coordination of care. Provide transformative services to individuals meeting the nursing facility level of care. Utilize a variety of assessments and screening tools to develop care plans. Seek to obtain respite and supplemental services to enrolled unpaid/family caregivers. Provide information and referral to community resources as well as long-term services and supports that best fit client needs. Facilitate caregiver support groups and caregiver classes. As appropriate, link caregivers to mental health services and other services deemed relevant and necessary. Engage in events to outreach to community partners.**

*****ESSENTIAL DUTIES & RESPONSIBILITIES includes but are not limited to the following:*****
** Facilitate completion of the Caregiver Survey.**
**Complete face to face TCARE assessments and Consultation Plans within required timeframe.**
**Provide information to the public about FCSP and KCSP services, including brochures, presentations and participate in the planning of the annual caregiver conference.**
**Provide unpaid caregivers and kinship families with assistance in gaining access to an array of appropriate services.**
**Develop, organize and facilitate caregiver support groups.**
**Organize and facilitate Powerful Tools for Caregivers Classes.**
**Provide unpaid caregivers and kinship families access to information that assists them in making decisions and solving problems related to their caregiver roles.**
**Complete documentation regarding units of service for all Core service areas, unmet needs and supplemental requests.**
**Provide advocacy as appropriate.**
**Provide timely referrals to appropriate caregiver related services.**
**Ensure a full understanding of the laws that govern child custody, power of attorney and guardianship and assist kinship families in accessing information about legal issues when they arise and provide support as they develop and pursue a plan of action.**
**Develop working relationships with key personnel within the criminal justice system and Spokane County schools.**
**Ensure an in-depth knowledge of community resources/services related to legal issues.**
**Complete Mini Assessment in GetCare for kinship families.**
**Perform FBH and ALTCEW charting requirements, correspondence, HRIS, and other paperwork in a complete and timely manner.**

****QUALIFIED CANDIDATES WILL HAVE:****

**A Bachelors degree in behavioral or health sciences and two years of paid on-the-job social service experience working with elderly and disabled adults OR a Bachelors degree and four years of paid on-the-job social service experience working with elderly and disabled adults.**
**Must apply for Agency Affiliation within 30 days of hire.**
**Must have reliable transportation, insurance and an active driver's license and be willing to use personal vehicle in the course of work.**

**Knowledge of elder care and disabled adult caregiving issues;**
**Ability to perform face-to-face assessments and screenings;**
**Proficiency with Microsoft Office suite and experience with database systems;**
