    Case Manager - Fort Lauderdale, United States - Staffing Now

    Description

    Summary/Objective: Ensure clients attend doctor appointments, have enough food, maintain housing, and pay bills. Coordinate with Housing Specialist regarding rental leases.

    Duties and Responsibilities:

    • Counsel clients on living skills like nutrition, money management, hygiene, and medication adherence.
    • Maintain accurate client records and county logs.
    • Assist with social security, retirement, and food stamp applications.
    • Conduct weekly calls and monthly visits to clients.
    • Review rental agreements with clients and assist with budgeting.
    • Coordinate essential purchases and donations for clients.
    • Respond to client emergencies and ensure compliance with program rules.
    • Comply with HIPAA Privacy Rules.

    Required Skills/Abilities:

    • Knowledge of substance abuse, mental illness, pharmacology, and homelessness.
    • Effective communication and interpersonal skills.
    • Ability to manage multiple cases and use de-escalation techniques.
    • Proficiency in Microsoft 365/Teams and record keeping.
    • Excellent organizational and time management skills.
    • Must pass background screening and drug test.

    Education and Experience:

    • Bachelor's degree in social work or related field with at least two years of related experience.
    • Knowledge of community resources preferred.
    • Must have a valid Florida Driver's License.

    Physical Requirements:

    • Periods of desk work and computer use.
    • Ability to conduct field work and client visits.
