- The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care.

- BSN
- Ohio RN License
- Case Management Certification (CMC or ACM) is required or obtained within 2 years of hire or eligibility
- BLS is required or obtained within 30 days of hire
- 3 years RN experience
- Conducting and documenting comprehensive assessments of patient's health status and psychosocial needs, including health literacy status and deficits, and ability to develop a collaborative plan with the patient and caregiver.
- Disease management and application of motivational interview techniques experience
- 1+ year of pediatric case management experience
- Utilization review/management experience with a working knowledge of InterQual criteria is strong preferred.
- Experience with funding sources, healthcare delivery services, clinical standards and outcomes management.
- Demonstrated ability to mitigate barriers to patient/family adherence, facilitate care transitions, and improve health outcomes in a pediatric setting.
- 5+ years of clinical experience in burn care and cleft lip and palate conditions.
- Bilingual in English and one additional language
