    The RN works directly under the supervision of the Nurse Manager and Assistant Nurse manager.

    As the Clinical Care Coordinator (CCC) for Nursing Mental Health, your work product is a direct reflection of the Nursing Service and is critical to fulfilling the mission of the VA.

    The following is to assure you are aware of your expectations as a representative and member of the Nursing Mental Health team.


    • Per local facility policy, the RN is responsible for maintaining RN licensure and Basic Life Support (BLS) certification.
    • Provide the best internal and external customer service by demonstrating interpersonal skills in a positive, tactful, and helpful manner.
    • Maintain confidentiality and discretion of the information communicated in your official capacity.
    • You are required to maintain a harassment-free work environment.
    • Encourage collaboration with all staff, interdisciplinary, and support services and maintain an environment conducive to teamwork and staff morale.
    • Your assignments should be completed prior to taking scheduled leave. Assignments not able to be completed due to time sensitive due dates are to be communicated to your supervisor and the Clinical Care Coordinator who will be covering you.
    • Engage in productive and efficient time management, updating your supervisor on barriers and other hindrances.
    • You are required to possess the knowledge required to: Collaborate, communicate, set priorities, and organize the work in order to meet deadlines, ensuring compliance with established processes, policies, and regulations.
    • Communicate tactfully and effectively, both orally and in writing, in order to meet program objectives. This may include preparing reports in various formats and presenting data to various organizational levels.
    • Possess advanced knowledge of survey readiness techniques for external survey bodies.
    • Possess advanced knowledge of medical terminology due to the technical nature of language utilized by clinicians.

    • Initiate, participate in and lead unit level performance improvement projects.

    Domiciliary-Standard work assignments:

    • Serve as the primary nursing point of contact with providers regarding the care of all residents in the MHRRTP (Mental Health Residential Rehabilitation Treatment Program).
    • Facilitate Interdisciplinary Team Meetings and document all updates from team members.
    • Serve as the primary liaison to the residents, their families; communicate their concerns to the interdisciplinary team.
    • Facilitate change of shift report, assure report starts on time and change of shift rounds are completed. Promote, participate, and evaluate patient/family health education.
    • Participate in interdisciplinary team meetings and share findings with team members. Follow-up on open consults as appropriate.
    • Contribute to the learning experiences of students. Provide education and direction of nursing personnel.
    • Gain expertise and actively participate in Shared Governance and performance improvement projects.
    • Consistently share information regarding performance improvement initiatives.
    • Review nursing documentation assessing for the need of any modifications to the care plan.
    • Promote improvements in the care of residents which maintain and enhance their dignity and respect, in full recognition of their individuality (e.g., assist Veterans with Perception of Care surveys).
    • Honor residents' rights to make choices about aspects of their lives related to ADLs (activities of daily living).
    • Tailor safe patient handling requirements for residents based on their physical needs.
    • Reviewing completion of documentation for admission, discharge, weekly assessments, and Safe Medication Management Policy (SMMP).
    • Complete regular audits of provider orders/care plan ensuring compliance with all active orders, according to RRTP guidance.
    • Serve as active member of the Screening Team.
    • Participate in Environment of Care Rounds as assigned to ensure a homelike, safe environment for residents.
    • Provide weekly update to nursing staff regarding changes to plans of care.
    • Monitor Care Plan for appropriate goals and outcomes.
    • Participate in nursing committees and national RRTP conference calls and report pertinent information to unit leadership and staff.
    • Identify additional areas for performance improvement.
    • Maintain a valid driver's license and DOT status for driving government vehicles.

    Work Schedule:
    Monday-Friday, Days, evenings, night (some compressed tours).

    Qualifications


    Basic Requirements:

    • U
    nited States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

    • English Language Proficiency: In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
    • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
    • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

    Grade Determinations:
    The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within the grade:


    • Nurse I Level I: An Associate Degree (ADN) or Diploma in Nursing, with no additional professional nursing required.
    • Nurse I Level II: An ADN or Diploma in Nursing AND 1 year of specialized nursing experience equivalent to Nurse I, Level 1; OR a Bachelor of Science in Nursing (BSN) with no additional professional nursing experience required.
    • Nurse I Level

    III:

    An ADN or Diploma in Nursing AND 2 years of professional nursing experience in which one year is equivalent to Nurse I, Level 2; OR a BSN and 1 year of professional nursing experience equivalent to the Nurse I, Level 2; OR a Master's degree in nursing (MSN) and no additional professional nursing experience; OR a Master's degree in a related field with a BSN and no additional professional nursing experience.


    • Nurse II: A BSN with 2 years of professional nursing equivalent to Nurse I, Level 3; OR an MSN with one year of specialized nursing experience equivalent to Nurse I, Level 3; OR a Master's degree in a related field with a BSN and one year of specialized nursing experience equivalent to Nurse I, Level 3; OR a Doctoral degree in Nursing with no professional nursing experience; OR a Doctoral degree in a related field with a BSN with no additional professional nursing experience.
    • Nurse

    III:

    MSN and 2 years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Master's degree in related field with BSN and two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctoral degree in Nursing with and one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctoral degree in a related field with a BSN and one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III.


    Note regarding MSN degrees:

    If your MSN was obtained via a Bridge Program that qualified for professional nursing registration, a BSN is not required.

    Physical Requirements:
    This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity.

    This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, pushing, and some lifting in the direct care of patients (35) pounds.

    Transferring patients and objects may be required. Mechanical inpatient lifts are provided.

    The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative or invasive procedures, or home settings.

    The incumbent may occasionally be exposed to patients who are combative to delirium, dementia, or psychiatric disorders.

    The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

    Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.

    The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
